Before submitting your completed application please print a copy for your signature. In order to complete the vendor registration process, you must return a copy of the signed application along with a completed W9 Form and a copy of your company’s business license to the following address:

Your application will be on file with Grady Health System for three years. After that time, it is important that your information be updated. Additionally, please forward a copy of your business license each time it is renewed.
