Frequently Asked Questions
What is the process to refer a patient to IDP?
Doctors or their designees may contact the HIV Discharge Coordinator Nurses at (404) 616-2426 or the TB Control and Intake Nurse at (404) 616-9712. Information can then be faxed to (404) 616-9700.
Are any forms needed to make a referral?
Previous medical records are required for the new patient intake appointment including discharge summary from the hospital, if available. TB clearance is required, therefore a recent TB skin test or other screening, or TB treatment record will enhance a rapid intake process. Other information should include a current problem list, medication list and history of antiretroviral therapy, recent labs or other diagnostic study reports.
